Twice a year is the standard advice. It is wrong for about half the properties in Johnston County, because a loblolly pine does not drop on the same schedule as an oak.
Spring and fall is the rule everybody quotes, and it was written for a neighborhood of deciduous trees that drop for six weeks and then stop. Half of the Triangle and Johnston County is not that. Loblolly pine sheds needles all year, with a heavy push in fall and a second one in spring, and pine straw is the worst thing that goes in a gutter.
Needles are small enough to slip past most guards, they mat into a dense felt that water cannot move, and they hold grit and pollen in place instead of letting it flush. A pine property with two cleanings a year is a property with clogged downspouts for a good part of the year.

Late May matters more than people think. That is when the pollen, the spring needle drop, and the shingle grit from winter have all settled into the trough together and turned into sludge, right before the summer storms arrive to test it.
Water that should have gone thirty feet out a downspout instead runs down the wall and into the soil at the footing. In clay soil that swells and shrinks with moisture, that is the expensive kind of problem.
A full trough holds standing water against the wood it is bolted to. Rotten fascia is the most common repair we see on properties that skipped a couple of years.
Overflow sheets down the wall and carries clay and tannin with it, staining the siding and the concrete below.
A sheet of water off a roofline strips mulch and cuts channels in beds every time it rains hard.
Uncommon here but not rare. A blocked trough that freezes and thaws works at the fascia and the shingle edge.
